Please ensure you read the below overview and requirements for this employment opportunity completely.Overview The role of the Social Worker is to create an environment in the home that provides opportunity for the community members to enjoy the highest possible quality of life and provides psychosocial support to residents, families, and partners, which will enhance the quality of life for residents and will promote the health of the intentional community of home.Shift: Full-TimeKnow each resident and their family including a psychosocial profile of the resident and an understanding of the family members and dynamicsParticipates in care planning for resident needsAssures that out of facility activities are available to residents.Understand and complete MDSs timely and accuratelyLead the Grievance Committee, hearing any resident concerns and following up on them timelyAbility to complete investigations with the assistance of the Executive DirectorProvide annual education on Abuse, Neglect, and MisappropriationIssue NOMNC, and ABN information and allow for appeal process.Support and encourage social work interns as neededIncludes families in the planning processUnderstands the process for Medicaid Waiver and helps support transitions of careEvaluates the effectiveness of the programs and activitiesWeekly support to each level of care in Independent Living, Assisted Living and NursingCompletes PAS/RR and LOCs on admissionDocuments in accordance with the organization's policiesComplete admission assessments and resident dischargesPerforms any other duties as assignedCase managementSkillsThe following are skills needed to perform, with or without reasonable accommodation, the essential functions of providing care to residents.
